I give my consent and approval to the participation of my child in the camp. I certify that my child is physically fit to take part in all activities. I release personnel and authorities from responsibilities in case of accident or injury.
I give permission for Thunder Bay Lakers to use the above-named candidate in a photo, video, or other digital media in any and all of it's publications, including web-based publications, without payment or other consideration.
